Hello, I’m Maria Gaines-Onwukwe.

As a board-certified Nurse Practitioner, I bring almost 19 years of experience caring for patients across Maryland, including 5 years in medical leadership roles,

I have cared for patients in hospitals, rehabilitation centers, intermediate care, emergency rooms, and more.

My diverse background in clinical practice and patient advocacy has equipped me to manage complex conditions, guide families through surgery and recovery, and provide personalized, compassionate care in every setting.

My philosophy is simple: treat every patient as if they were my own family with dignity, compassion, and respect.

Who We Are

  • CCAYD was founded with a vision: healthcare that doesn’t stop at the clinic door.

As a subsidiary of Pearldahlia, LLC, we saw how families struggled to balance work, caregiving, and medical needs, and how patients longed for care that felt more human.

That’s why we created a concierge model of healthcare blending clinical excellence with genuine compassion, so every patient feels seen, heard, and cared for.

  • We exist to make healthcare more personal.

Too often, families are overwhelmed with appointments, travel, and the stress of navigating the system.

At CCAYD, we bridge that gap by bringing nurse practitioner-led care directly to you.

Whether it’s managing medications, preparing for surgery, or recovering after a procedure, we make sure every visit feels supportive, reliable, and centered on your well-being.

    1. Every patient deserves kindness and respect.

    2. Every patient deserves professional, evidence-based medical care.

    3. Every patient deserves accessible care in the comfort of their home, without waiting rooms.

    4. Every patient deserves a personalized care plan that is made to work with you, your family, and your providers.
