Who We Are

  • CCAYD was founded with a vision: healthcare that doesn’t stop at the clinic door.

As a subsidiary of Pearldahlia, LLC, we saw how families struggled to balance work, caregiving, and medical needs, and how patients longed for care that felt more human.

That’s why we created a concierge model of healthcare blending clinical excellence with genuine compassion, so every patient feels seen, heard, and cared for.

  • We exist to make healthcare more personal.

Too often, families are overwhelmed with appointments, travel, and the stress of navigating the system.

At CCAYD, we bridge that gap by bringing nurse practitioner-led care directly to you.

Whether it’s managing medications, preparing for surgery, or recovering after a procedure, we make sure every visit feels supportive, reliable, and centered on your well-being.

    1. Every patient deserves kindness and respect.

    2. Every patient deserves professional, evidence-based medical care.

    3. Every patient deserves accessible care in the comfort of their home, without waiting rooms.

    4. Every patient deserves a personalized care plan that is made to work with you, your family, and your providers.

  • We provide integrated medical management with a personalized, patient-centered approach.

Our model blends clinical expertise with compassionate delivery, offering both telemedicine and in-person services based on clinical appropriateness.

We focus on continuity, accessibility, and thoughtful medical oversight, ensuring that patients receive coordinated care that addresses chronic conditions, acute concerns, behavioral health needs, and supportive therapies in one integrated setting.

  • Hybrid Integrated Medical Management Care is delivered through a combination of secure telemedicine and in-person visits.

This flexible structure allows us to provide ongoing monitoring, timely follow-ups, and appropriate hands-on services when needed.

    We specialize in:

    • Ongoing medical management of select chronic conditions

    • Medical weight management

    • Integrated behavioral health (medical oversight and coordination)

    • Low-acuity urgent medical visits

    • Pre-operative medical clearance (limited scope)

    • Supportive IV therapy as an adjunct to treatment

Our services are designed to complement, not replace, care provided by a patient’s primary care provider.
